We’ve put together some helpful information about the services available on the Single Point of Access.

Clinical Assessment Service (GP Out of Hours)
– Option 1

The Clinical Assessment Service can offer the following support:

    •  Give medical advice
    •  Support people who may have fallen in their home
    •  Help those that are housebound and unwell
    •  Signpost and refer to appropriate services
    •  Answer calls with a local clinician on the line

Focus Independent Adult Social Work
– Option 2

Focus provides the adult social care and safeguarding adult services in North East Lincolnshire on behalf of North East Lincolnshire Council.

We offer you advice and information on:

      • Support at homeand keeping safe at home
      • Support for unpaid carers of adults
      • Paying for care
      • Residential and respite care
      • Sensory impairment support
      • Community therapy referrals
      • Extra care housing
      • Adult day care services
      • Supported activites
      • Signposting to local services
      • Adult safeguarding concerns.

Navigo Mental Health Services
– Option 3

Navigo offers a range of free mental health and wellbeing support services

      • Call the mental health team on 256 256 (Option 3) or NHS111
      • Walk in to Harrison House on Peaks Lane (24 hours a day)
      • Visit safesapce at Navigo House (for opening times visit the Navigo Website)
      • Text ORANGE to 85258 (24 hours a day)
      • If you’re in immediate physical danger or have a medical emergency, call 999

Hospital Discharge Team
– Option 4

If you are an inpatient in hospital, the Hospital Discharge Team will provide support and guidance to help you make a successful and safe move back into your own home or an alternative placement (eg, rehabilitation or discharge to assess). The Hospital Discharge Team provide the following services:

      • Combined review of health and social care needs
      • Direct access to Recovery and Recuperation short stay placements
      • All individuals will have a case manager who will be responsible for their discharge out of hospital, through the recovery and recuperation service. Onward referral to the following: (depending on the patient’s pathway from hospital)  Focus, Intermediate Tier Therapy, Intermediate Care at Home, CPG (Cambridge Park) and Continuing Health Care (CHC).
      • Reinstating, new or increased packages of care
      • Provide and offer advice and guidance about other community services and signpost you to other services
      • Arranging equipment to enable safe discharge
      • Close liaison with community services.

Connect NEL
– Option 5

Connect NEL is a helpful free signposting service available Monday to Friday 9am to 5pm.

This FREE service is available for all and the team can offer you advice and information on a range of local services, including weight management, anxiety , debt and employment.
